Here is addition...

Nature of disability: nightly vent dependent, due to post polio syndrome
since 1994. Contracted polio at the age of 7 mths old...due to the vaccine.
I use a PLV-100 vent. I have a size 6 cuffless Shiley trach. Use a power
wheelchair for mobility.
